At The Dental Practice, we strive to be as efficient as possible when it comes to your treatment plan. Your time is important to us. To help you receive the highest quality of care, we’ve provided the following information for our new patients to review.

We are currently accepting new patients. Please call our office at 641-792-2780 to schedule an appointment. If you’re unable to keep your scheduled appointment time, please call us with a 24-hour notice to reschedule.
What to Bring to Your First Appointment
To provide you with prompt treatment, please bring the following items with you for your initial visit:
- Current medical information
- Current medicine list
- Current insurance information
Please arrive 15 minutes before your scheduled appointment time to fill out the necessary paperwork. If you would like to complete your paperwork beforehand, you may print and fill out the appropriate forms found below.
We ask that you realize that we do NOT work for an insurance company. Rather we work 100 percent for our patients. We feel that insurance can be a great benefit for many patients and want you to know we will do everything in our power to ensure you get every benefit allotted in your insurance contract. However, the treatment we recommend and the fees we charge WILL ALWAYS BE BASED ON YOUR INDIVIDUAL NEEDS, NOT YOUR INSURANCE COVERAGE.
For your convenience, our office accepts cash, checks, Visa, Discover, MasterCard, American Express, & CareCredit.
Patients over 65 will receive a 5% bookkeeping allowance on all fees over $100 that are paid in full on the day of treatment or at the beginning of a treatment series.
